Why is it that, when someone is pleading not criminally responsible reason by insanity...?

Even though being not guilty by reason of insanity happens a lot on television and in the movies, it rarely ever happens in real life. Simply being insane is not sufficient to prevent criminal penalties. A person has to be incapable of telling right from wrong and the jury has to believe it.
